A medical grade chemical peel and a day-spa peel are not the same treatment at two price points — they are different categories of intervention, separated by depth, chemistry, credentials, and consequences.
What Actually Defines a Medical Grade Chemical Peel
The term isn’t marketing. In the dermatologic literature, peels are classified by exactly one variable: depth of controlled injury. This is not an at-home treatment and definitely worth investing the time and research to find the right licensed care provider for you.
Superficial peels act within the epidermis, the outermost layer. Medium-depth peels reach the papillary dermis and upper reticular dermis. Deep peels go further beyond. That framework, set out by the International Peeling Society in the Journal of the American Academy of Dermatology, defines the following clinical applications.¹

The Deepest Chemical Peel is NOT Standard Practice
A spa peel is intentionally designed to stay in the epidermis. Superficial peels genuinely improve texture, dullness, and mild congestion, and they’re low-risk by construction.
But pigment sitting in the dermis, textural scarring, and photoaging damage below the surface cannot be reached by a treatment that never travels there. No number of surface peels adds up to a dermal result.

Concentration and Formulation Are Clinical Decisions
Superficial peels typically use alpha hydroxy acids like glycolic and lactic, beta hydroxy acids like salicylic, Jessner’s solution, or trichloroacetic acid at low concentration.¹
Medium-depth work is different chemistry. The reference technique in the literature pairs a keratolytic priming agent with TCA at higher concentration — Jessner’s solution followed by 35% TCA, described by Monheit in Dermatologic Clinics, where pretreatment degreasing and combination chemicals allow deeper, more controlled penetration.²
Concentration alone doesn’t tell the whole story either. pH, vehicle, contact time, number of passes, and pressure all change how far a solution travels. Two bottles labeled the same percentage can behave entirely differently on skin. Which is why a medical grade chemical peel is judged by clinical endpoint — the frost, the erythema, the tissue response — not by the number on the label.
Who Performs a Chemical Peel
Licensed estheticians are trained and authorized to work on the surface of the skin. That’s a real and valuable skill set, and it has a defined boundary.
Treatments that create controlled injury below the epidermis fall under medical practice. They require a licensed medical provider’s appropriate supervision, and a clinical assessment before treatment begins. A medical grade chemical peel is, in regulatory terms, a delegated medical act — not a spa service.

The Consultation Comes First — Always
Before a medical grade chemical peel, we assess skin type, history of hyperpigmentation, active infection, recent isotretinoin use, sun exposure, current medications, and scarring tendency.
This is procedure. Post-peel hyperpigmentation is a frequently encountered complication of chemical peeling, and the risk is markedly higher in more deeply pigmented skin.³ It’s managed before any solution touches skin, through skin typing, priming, and medically controlled depth selection.
A spa peel usually begins with an intake form. A medical peel begins with an examination.
Prep Starts Weeks Before the Appointment
Skin should be prepared, not shocked.
Preparation for a medical grade chemical peel often begins two to four weeks out. In a double-blind randomized trial of fifty melasma patients, Nanda and colleagues primed one group with 2% hydroquinone and one with 0.025% tretinoin for two weeks before TCA peeling. At the twenty-four-week follow-up, the hydroquinone-primed group held their results better and showed less post-peel reactive hyperpigmentation.³
Priming thins the stratum corneum so the peel penetrates evenly, and it suppresses melanocyte activity so pigment doesn’t rebound afterward. Uneven prep produces uneven results. It’s the least glamorous phase of the process and the one that most determines the outcome.
What Happens Underneath the Skin
Surface exfoliation makes skin look brighter. Controlled dermal injury resurfaces skin completely and evenly.
In a double-blind, vehicle-controlled study of forty-one volunteers published in Dermatologic Surgery, Newman and colleagues treated photoaged skin with 50% glycolic acid weekly for four weeks and took punch biopsies at five weeks. Histology showed thinning of the stratum corneum, granular layer enhancement, and epidermal thickening, with increased collagen thickness in some dermal specimens. Clinically: less rough texture, fewer fine wrinkles, fewer solar keratoses, and lightening of solar lentigines.⁴
Deeper agents push further. A 2021 systematic review of trichloroacetic acid peeling for photoaging found that low concentrations effectively address superficial sun damage, while medium-depth protocols using higher TCA concentrations, or TCA in combination, function as genuine resurfacing agents for wrinkles.⁵
Translation:
a medical grade chemical peel doesn’t only remove damaged tissue. It initiates a repair cascade of fibroblast activation, new collagen synthesis, and reorganized dermal architecture that continues for months after the visible peeling stops.

Every medical grade chemical peel carries a risk profile. While rare, prolonged erythema, pigment shifts, infection, and with depth, scarring are all possible risks.
Medical settings are staffed to recognize and treat complications. Antiviral prophylaxis where indicated, topical steroids, pigment-suppression protocols, and structured follow-up are established, not improvised after damage is done.
